Support Coordination

Xtremecare Australia is an NDIS Registered Provider, who is committed to delivering a culturally appropriate Support Coordination service, our experienced Support Coordination team understands the NDIS reform and what this means for our Indigenous participants living in rural and remote communities.

As an NDIS participant, it is important that you understand your NDIS Plan, your decisions, and your goals. Our Xtremecare Support Coordination staff will take you through your NDIS plan and explain what your Core Supports, Capacity Building, and Capital budget areas mean, and what services and supports you can access to assist you in achieving your goals. We build trusting relationships with our participants and encourage active participation throughout the implementation of your NDIS plan.

 

 
 

XtremeCARE DELIVERING A CULTURALLY SAFE SERVICE

Xtremecare Australia offers a culturally appropriate and safe service to Aboriginal and Torres Strait Islander NDIS participants, living in rural and remote communities, through language and cultural connection.

Our goal is to ensure that individuals, families and communities feel comfortable and understand our services. In remote communities, the language, culture and country can differ, so we need to accommodate these differences and tailor a service that is appropriate to the needs of the participant and their community.  

 
 

NDIS Plans

Support Coordination (if required) is included in the Capacity Building budget or your NDIS Plan. This is a fixed amount for strengthening participant’s abilities to coordinate and implement supports in their plans and to participate in the community.

core supports

Core Supports help you with everyday activities, your current disability-related needs and to work towards your goals. Your Core Supports budget is the most flexible.

capacity building

Capacity Building Supports help build your independence and skills to help you pursue your goals. This budget area covers supports, such as Allied Health services.

capital 

Capital Supports include higher-cost pieces of assistive technology, equipment and home or vehicle modifications, and funding for one-off purchases you may need (including Specialist Disability Accommodation),
